We are seeking a Nurse Practitioner or Physician Assistant to join our growing Primary Care team. Our practices bring together board-certified physicians and advanced practice providers experienced in both adult and pediatric medicine, with a shared commitment to providing compassionate, patient-centered care.

As part of our team, you’ll work collaboratively with primary care providers and specialists to deliver coordinated, high-quality care while enjoying a meaningful level of autonomy in managing your own patients.

We are currently growing our team at the following locations:

• Primary Care - Wilton: 3065 NY-50 Unit 30, Saratoga Springs, NY

• Primary Care - Milton: 510 Geyser Rd, Ballston Spa, NY

• Primary Care - Saratoga: 381 Church St, Saratoga Springs, NY

Call is 1:15, shared with providers.

As a Nurse Practitioner or Physician Assistant, you will evaluate, diagnose, and manage patients while working closely with physicians, APPs, and other members of the care team.

Responsibilities include:

• Obtain, document, and update patient medical histories

• Perform physical examinations and conduct preventive screening procedures based on age and history.

• Identify patient health risks and clinical needs

• Develop differential diagnoses and individualized plans of care

• Order and review diagnostic testing

• Prescribe and manage appropriate pharmacologic therapies

• Evaluate patient outcomes and adjust treatment plans as needed

• Coordinate referrals to specialists and community resources

• Facilitate admission or discharge to the appropriate level of care when indicated

• Collaborate in team meetings and interdisciplinary care discussions

• Participate in quality improvement initiatives and the development of standards of care

• Maintain current clinical knowledge and professional competencies

Qualifications:

• Current New York State license to practice as a Nurse Practitioner or Physician Assistant

• Current required professional credentials and certifications, including applicable licensure, BLS/ACLS, infection control, and DEA registration

• Commitment to collaborative, compassionate, patient-centered care

• Experience with Epic is preferred, but not required
