Description

The Care Navigator fulfills the Integrated Care Navigator function identified in Legacy's approved SBIRT staffing plan and provides non-clinical care coordination and referral support for consumers participating in the SBIRT project. The Care Navigator coordinates referrals and warm handoffs to behavioral health, substance use treatment, primary care, recovery support, and community resources; assists with scheduling and access; tracks referral status; follows up to confirm linkage; and works with consumers and the interdisciplinary team to address barriers to engagement or continuity of care. The Care Navigator supports outreach, maintains current referral resources, and documents referral, linkage, follow-up, transportation, and barrier-resolution activities in the electronic health record and project tracking systems. This is a navigation and coordination role and does not independently provide diagnosis, clinical assessment, psychotherapy, or nursing services. The primary assignment during time charged to the SBIRT award is SBIRT navigation, linkage, follow-up, and implementation support. Outpatient navigation or coordination duties may be performed during SBIRT-funded time when they directly support SBIRT participants, referral completion, project integration, outreach, or sustainability.

Legacy’s Screening, Brief Intervention, and Referral to Treatment (SBIRT) Program is funded through a grant from the Substance Abuse and Mental Health Services Administration (SAMHSA). The program provides early identification and intervention for substance use concerns for individuals age 12 and older, with a particular focus on youth and young adults ages 12–21. The program operates across Atlantic, Burlington, and Camden Counties and integrates SBIRT into outpatient behavioral health, healthcare, school, mobile outreach, and community-based settings.

Services include evidence-based substance use screening, brief intervention and brief treatment when indicated, and referral to treatment for individuals needing more intensive substance use services. The program also provides care navigation, warm handoffs, follow-up, peer and recovery support, and coordination with behavioral health, primary care, substance use treatment, and community resources to reduce barriers and improve continuity of care.

Hours:

Position is full time and includes the following:

Qualifications

Education & Experience:

High school diploma or equivalent required. Associate's degree or prior administrative, referral, scheduling, care coordination, or navigation experience in a behavioral health or healthcare setting preferred. Strong organizational, communication, customer-service, and follow-through skills required. Ability to manage multiple priorities and accurately track referrals, appointments, follow-up attempts, barriers, and outcomes. Proficiency with scheduling systems, Microsoft Office, electronic health record software, and electronic data entry required. Must understand role boundaries, confidentiality, authorization for information sharing, and escalation of clinical or safety concerns to qualified staff. Familiarity with behavioral health, substance use treatment, primary care, transportation, insurance/benefit, and community resource systems preferred. Must possess a valid driver's license, be able to drive an Agency-owned vehicle including a mobile outreach van, and travel between approved service sites. Position may include consumer transportation when assigned and authorized under Agency policy.
