BAYADA Home Health has an immediate opening for a Full-Time LPN, Licensed Practical Nurse to perform home health visits . This office provides exceptional home health medical visits to adult and geriatric patients in Pembroke, Marshfield, Hanover and surrounding towns . Prior clinical experience as a Licensed Practical Nurse is required.

Licensed Practical Nurse (LPN) Responsibilities:

• Making home visits to clients living in designated territories.

• Performing assigned duties, including administration of medication, wound care, treatments, and procedures.

• Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager.

• Following up with, executing, and properly documenting doctors' orders.

• Performing client assessments as necessary.

• Case management and coordination.

• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a touch pad tablet.

Qualifications of a Licensed Practical Nurse (LPN):

• A current license as a Licensed Practice Nurse in the state of Massachusetts.

• Graduation from an accredited and approved nursing program.

Compensation & Schedule:

• Competitive pay ranges with a weekly pay schedule

• Flexible scheduling

Pay: Weekly

Base Pay, depending on qualifications and subject to negotiations

$42.00 - $48.00 per point
