Job Summary

The Clinical Validation Registered Nurse (RN) is responsible for reviewing clinical documentation and validating that documented diagnoses accurately reflect the patient’s clinical condition, treatment, and overall medical record. This role partners with physicians, nursing staff, Health Information Management (HIM), Clinical Documentation Integrity (CDI), Coding, Quality, and other interdisciplinary teams to identify opportunities for improved clinical documentation and accurate representation of patient acuity and severity of illness.

The Clinical Validation RN applies clinical expertise, critical thinking, and knowledge of coding and documentation guidelines to evaluate medical records, identify potential inconsistencies or unsupported diagnoses, and facilitate appropriate clarification of the clinical record. The position supports accurate coding, quality reporting, reimbursement, compliance, and meaningful representation of patient outcomes.

Essential Functions of the Role

  • Conduct comprehensive clinical validation reviews of inpatient and/or outpatient medical records to determine whether documented diagnoses are clinically supported by the patient’s condition, diagnostic findings, treatment, and clinical indicators.

  • Review medical record documentation, including physician notes, nursing documentation, laboratory results, diagnostic testing, medications, procedures, and other relevant clinical information.

  • Identify diagnoses that may lack sufficient clinical support, as well as opportunities for clarification of conflicting, incomplete, or inconsistent documentation.

  • Develop and issue compliant, non-leading clinical documentation clarification queries in accordance with organizational policies and applicable regulatory, coding, and documentation guidelines.

  • Collaborate with CDI specialists, physicians, coders, HIM professionals, and other members of the care team to promote accurate and complete clinical documentation.

  • Apply current clinical knowledge and established clinical criteria to support validation decisions.

  • Distinguish between clinically supported diagnoses and diagnoses that may not be supported by the available medical record.

  • Maintain working knowledge of ICD-10-CM/PCS coding concepts, official coding guidelines, Medicare/CMS requirements, payer requirements, and applicable clinical documentation standards.

  • Monitor trends in clinical validation opportunities and communicate findings to appropriate leadership and interdisciplinary teams.

  • Participate in education and feedback activities for physicians, advanced practice providers, nurses, CDI staff, and coding professionals as appropriate.

  • Maintain accurate and timely documentation of review activities, queries, outcomes, and follow-up.

  • Support organizational initiatives related to clinical documentation integrity, quality, compliance, reimbursement, and accurate severity-of-illness reporting.

  • Maintain confidentiality of protected health information and comply with HIPAA and all applicable organizational policies and regulatory requirements.

  • Perform other duties and responsibilities as assigned.

Key Success Factors

Success in this role is demonstrated through:

  • Clinical judgment: Uses sound nursing knowledge and critical thinking to assess the clinical validity of diagnoses.

  • Attention to detail: Thoroughly evaluates the medical record and recognizes discrepancies, inconsistencies, and documentation gaps.

  • Knowledge of documentation and coding: Demonstrates understanding of clinical documentation principles, coding concepts, and regulatory requirements.

  • Effective communication: Communicates professionally and collaboratively with physicians, clinicians, CDI, coding, HIM, and leadership.

  • Query quality: Develops clear, objective, clinically supported, and compliant documentation clarification queries.

  • Analytical ability: Interprets clinical indicators and synthesizes information from multiple areas of the medical record.

  • Compliance focus: Maintains an objective approach and ensures clinical validation activities are consistent with regulatory, ethical, and organizational standards.

  • Productivity and organization: Manages assigned reviews efficiently while maintaining accuracy and quality.

  • Continuous learning: Keeps current with changes in clinical practice, coding guidelines, payer requirements, regulatory expectations, and clinical validation standards.

  • Teamwork: Builds effective relationships across clinical and administrative departments to support organizational goals.

Preferred Qualifications:

  • Current, unrestricted Registered Nurse (RN) license in the applicable state(s).

  • Bachelor of Science in Nursing (BSN) preferred.

  • 3+ years of clinical nursing experience, preferably in an acute-care hospital setting.

  • Strong understanding of clinical conditions, disease processes, diagnostic testing, treatment plans, and patient acuity.

  • Demonstrated ability to independently review and interpret complex medical records.

  • Strong written and verbal communication skills.

  • Proficiency with electronic medical records and Microsoft Office or comparable applications.

  • Ability to work independently, prioritize multiple assignments, and meet established productivity and quality expectations.

  • Experience in Clinical Documentation Integrity (CDI), clinical validation, coding, utilization review, case management, or Health Information Management.

  • CCDS, CDIP, or other relevant clinical documentation/coding certification.

  • Experience with inpatient coding and/or ICD-10-CM/PCS concepts.
