AHMG Health Optimization Center at Celebration 1530 Celebration Blvd, Suite 300A, Celebration, FL 34746 Monday - Friday 8am - 5pm

• Prioritizes whole person health problems, formulates goals, and constructs therapeutic plans, revising as needed based on continuous evaluation and assessment of progress.
• Quickly appraises crisis situations and selects appropriate interventions.
• Mediates highly complex situations and develops treatment plans with minimal supervision.
• Acquires working knowledge of motivational interviewing and works with complex clients.
• Seeks appropriate consultation when necessary.
• Addresses participants' behavioral health as it contributes to chronic disease.
• Identifies modifiable social determinants and works with participants to modify them.
• Collaborates with referring physicians to navigate participants through an integrated referral network to identify necessary resources for physical and behavioral health diagnoses.
• Communicates progress of participants to referring physicians.
• Evaluates practice upon completion of case intervention to determine success and achievement of expected outcomes.
• Prepares and teaches program classes.
• Identifies necessary resources for patient referral to achieve whole-person health and increased stability in daily living.
• Performs other duties as assigned.

• Proficient in Microsoft Office applications of Word, Excel, PowerPoint, and Outlook, as well as Internet knowledge and skills [Required]
• Knowledge and understanding of CREATION Health framework [Preferred]

Education:

• Master's [Required]

Field of Study:

• in Behavioral Health, Psychology, Social Work or a related field

Work Experience:

• 2+ nursing, social work, or related experience in the clinical setting [Required]
• Experience in outpatient or home health setting [Preferred]

Licenses and Certifications:

• Clinical Social Worker License (LCSW) [Required] OR Licensed Mental Health Counselor (LMHC) [Required]

Pay Range:

$54,766.40 - $101,878.40 Background Screening Requirement (Florida Law)
