About the Role

As part of a high-functioning leadership and care team, the MDS Coordinator / Case Mix Nurse conducts and coordinates the development and completion of the resident assessment in accordance with the rules and regulations of state and federal government and the policies and goals of the facility. Reporting directly to the Director of Nursing, you will play a pivotal role in maintaining case mix validity, ensuring regulatory compliance, and championing an atmosphere of teamwork, hospitality, and comfort for our residents.

Key Responsibilities

  • Assessment & RAI Process: Assess and oversee the assessment of all residents through the RAI process, ensuring coding accuracy and capturing individual care needs and levels of assistance.

  • Care Planning & Review: Review, oversee, and complete care plans on a regular basis for accuracy, appropriateness, and case mix validity, adhering to standards of nursing practice (including nursing restorative plans).

  • Interdisciplinary Coordination: Establish documentation approaches, instruct nursing and ancillary staff on proper documentation, and communicate MDS due dates so interdisciplinary team members complete their portions on time.

  • Education & Guidance: Instruct all disciplines in proper procedures, completion of MDS items, CAAs, case mix payment systems, and Quality Indicators.

Qualifications & Requirements

  • Licensure: Current, active Maine RN licensure or current, active Maine Licensed Practical Nurse (LPN).

  • Experience & Certification: MDS and ICD-10 coding experience preferred; RAC-CT certification preferred. Strong clinical assessment, care planning, and delivery of care skills within a long-term care population.

  • Knowledge & Skills: Working knowledge of geriatric healthcare needs, federal and state long-term care regulations, Medicare and MaineCare rules (including the State Operations Manual and Principles of Reimbursement), MDS manuals, and Quality Indicator systems.

  • Technical & Professional Proficiency: Strong computer literacy, including experience with electronic medical records and Microsoft Office (Word, Excel), coupled with clear communication and the ability to work tactfully with residents, families, staff, physicians, regulators, and the community.
