As a Registered Nurse, you will coordinate and oversee patients' home care while collaborating with care teams to develop and implement individualized care plans. You will assess patients, provide skilled nursing care, educate families, maintain timely documentation, and communicate progress to the clinical manager, physician, and care team.

Primary Responsibilities:

• Coordinates and oversees all aspects of patients' home care

• Collaborates with patient care teams to develop and implement care plans

• Educates families on patient condition, needs and care

• Performs head to toe physical assessments

• Checks vital signs and manages medications

• Gives injections, IV therapy or tube feedings

• Administers and maintains catheters

• Completes documentation timely and accurately

• Supervises LPNs and HHAs

• Regularly communicates patient progress to the clinical manager, physician and care team

• Other duties as assigned

$10,000 Sign-on Bonus for External Candidates

Required Qualifications:

• Current RN license, specific to the state(s) you are assigned to work

• Current CPR certification

• Valid driver's license, reliable transportation and liability insurance

*Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.

Preferred Qualifications:

• 1+ year of clinical experience as a registered nurse (RN)

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $40.35 to $49.30 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
