Employment Type: Full timeShift: Rotating Shift Description: Mount Carmel St. Ann’s is seeking a Clinical Pharmacist to join our Pharmacy team, with a focus on supporting the Internal Medicine units, Emergency Department and central pharmacy operations. This evening-shift role offers the opportunity to practice at the top of your license in a collaborative, multidisciplinary environment dedicated to delivering exceptional patient care.
As a trusted medication expert, you will work closely with physicians, nurses, medical residents, and other care team members to optimize medication therapy and improve patient outcomes. In addition to direct patient care responsibilities, you will have opportunities to mentor and educate Pharmacy Interns, APPE students, and medical residents while contributing to quality improvement and operational initiatives.
The ideal candidate is passionate about patient care, teaching, precepting, and continuous process improvement. Experience in Internal Medicine and/or Critical Care Pharmacy is strongly preferred. Completion of a PGY1 Residency or at least three years of hospital pharmacy experience is preferred.

Shift: Evening Position Purpose
The Patient Care Pharmacist, within the scope of the Ohio Pharmacist Practice Act, will provide timely, accurate, cost effective, and patient-centric pharmaceutical care.
What you will do:

Coordinates the preparation, distribution, and dispensing of medication as appropriate in compliance with state and federal regulations and laws, accreditation standards, and departmental policies and procedures; directs and assists pharmacy support staff.

Maintains personal compliance and assists in maintenance of departmental compliance with continuous regulatory readiness. Also maintains records related to the transactions of the pharmacy, to achieve adequate control and accountability for all drugs, including controlled substances.

Assists in the management of pharmacy inventory by providing information to pharmacy purchasing, removing expired contaminated or deteriorated stock when detected, helping maintain perpetual inventories, and processing non-formulary requests.

Interprets, evaluates and verifies all medication orders.

Collaborates with multidisciplinary team in order to assure optimal and cost effective drug therapy. This includes adjusting drug dosing and schedules, recommending the most efficacious therapy, practicing stewardship in antibiotics, performing drug consults and monitoring, and detecting possible adverse drug reactions, food/drug interactions, drug/drug interactions, and allergic cross reactivity.
Minimum Qualifications:

Education: Bachelor or Doctor of Pharmacy required

Licensure / Certification: Current license to practice as a Registered Pharmacist in the State of Ohio.

Experience: PGY1 residency or three years of institutional pharmacy experience in an advanced clinical setting is preferred

Demonstrates the ability to plan, organize and manage pharmaceutical care, including delegating tasks to and providing direction to other members of the pharmacy team

Performs work independently with minimum supervision, and is effective in a stressful environment
