Join our pharmacy team as a Patient Care Pharmacist and play a vital role in delivering exceptional patient care in a dynamic acute care setting. This position partners closely with interdisciplinary healthcare teams to optimize medication therapy for critically ill and complex patients while advancing evidence-based pharmacy practice.

In this role, you will provide patient-centered pharmaceutical care, ensure safe and effective medication management, and contribute to outstanding clinical outcomes through collaboration, clinical expertise, and operational excellence. You will also have the opportunity to mentor and develop the next generation of pharmacy professionals by serving as a preceptor for pharmacy residents and students.

Key Responsibilities:

  • Provide comprehensive clinical pharmacy services and optimize medication therapy for complex patient populations.
  • Collaborate with physicians, nurses, and other healthcare professionals to support evidence-based patient care.
  • Perform medication order verification, therapeutic drug monitoring, antimicrobial stewardship activities, and clinical consultations.
  • Identify and resolve medication-related issues, including adverse reactions, drug interactions, and dosing concerns.
  • Precept and mentor pharmacy residents, students, and pharmacy staff.
  • Ensure compliance with regulatory standards, medication safety practices, and controlled substance requirements.
  • Support pharmacy operations, inventory management, and formulary stewardship initiatives.

  • The Patient Care Pharmacist, within the scope of the Ohio Pharmacist Practice Act, will provide timely, accurate, cost effective, and patient-centric pharmaceutical care.

What You Will Do:

  • Coordinates the preparation, distribution, and dispensing of medication as appropriate in compliance with state and federal regulations and laws, accreditation standards, and departmental policies and procedures; directs and assists pharmacy support staff.

  • Maintains personal compliance and assists in maintenance of departmental compliance with continuous regulatory readiness. Also maintains records related to the transactions of the pharmacy, to achieve adequate control and accountability for all drugs, including controlled substances.

  • Assists in the management of pharmacy inventory by providing information to pharmacy purchasing, removing expired contaminated or deteriorated stock when detected, helping maintain perpetual inventories, and processing non-formulary requests.

  • Interprets, evaluates and verifies all medication orders.

  • Collaborates with multidisciplinary team in order to assure optimal and cost effective drug therapy. This includes adjusting drug dosing and schedules, recommending the most efficacious therapy, practicing stewardship in antibiotics, performing drug consults and monitoring, and detecting possible adverse drug reactions, food/drug interactions, drug/drug interactions, and allergic cross reactivity.

Minimum Qualifications:

  • Education: Bachelor or Doctor of Pharmacy required

  • Licensure / Certification: Current license to practice as a Registered Pharmacist in the State of Ohio.

  • Experience: PGY1 residency or three years of institutional pharmacy experience in an advanced clinical setting is preferred

  • Effective Communication Skills

  • Demonstrates the ability to plan, organize and manage pharmaceutical care, including delegating tasks to and providing direction to other members of the pharmacy team

  • Performs work independently with minimum supervision, and is effective in a stressful environment
