Description

We are seeking a detail orientated and dependable Registered Nurse to join our team!

The Registered Nurse works one on one with clients through Clarity Care’s Home Health Department; focusing on their individual needs to help them achieve improved quality of life and a sense of independence.

Ideal Candidate Profile

  • Licensed Registered Nurse (RN)

  • Experience with disability services preferred

  • Strong clinical assessment skills

  • Knowledge of state healthcare regulations

  • Compassionate, person-centered approach

  • Effective verbal and written communication

  • Skilled in care planning

Key Responsibilities:

  • Promotes and encourages the organization’s mission, vision, values, and philosophy.

  • Adheres to the organization’s code of conduct, standards, policies, procedures, and appropriately brings to management’s attention actual and potential violations.

  • Acts with integrity and maintains a professional image within the organization and throughout the community.

  • Acts as a team member within the department and organization to assist in accomplishing goals and objectives.

  • Maintains good attendance on the job and attends continuing education, training and meetings as scheduled.

  • Cooperates with supervisors and all levels of management involved in evaluating, delegating, and improving performance.

  • Exercises good judgment, decision making, and regard for safety within all aspects of performing job duties.

  • Communicates effectively, efficiently and ensures all information is transmitted appropriately. Understands, gives, reads, and follows written, verbal, or demonstrated instruction.

  • Document and report any medication, safety issues, and significant events following all documentation policies.

  • Appropriately requests and extends assistance, learns, prioritize tasks, independently assess situations, problem-solves, multi-tasks, and appropriately responds to challenges.

  • Achieves expected outcomes to meet organizational goals.

  • Possess problem-solving skills and apply them to a wide range of situations promptly, appropriately, and safely.

  • Demonstrate language, mathematical and literacy skills that show the ability to communicate, understand, give, read, and/or follow written, verbal, or demonstrated instruction, write information, request and extend assistance.

  • Possess leadership and people skills to plan, organize, staff, motivate, direct and control the work and any workers that might be assigned.

  • Possess ability to work cooperatively in a team environment.

  • Possess physical aptitudes and capabilities that will enable performance of the job to include: ability to stoop, stand, pull, push, hear, see, kneel, walk, grasp, reach, talk, and lift at least 35 pounds.

  • Performs all other duties as assigned.

Education and Qualifications

  • Graduate of an accredited College of Nursing with a Bachelor’s Degree in Nursing preferred.

  • Current Registered Nurse Licensure with the Wisconsin State Board of Nursing Required.

  • Undergo successful orientation and participate in competency-based assessments of skills on a regular basis as required by state law.

  • One year experience in General, Home Health and/or Public Health nursing required, 2 years and Home Health experience preferred.

  • Must possess a valid WI driver’s license (where required), and ability to meet agency driving standards. Must provide proof of auto insurance (where required).

  • Must pass a periodic WI Caregiver Background Checks including Department of Justice Criminal Record Checks, and/or that of previous states of residence.
