Overview

Schedule: Monday-Friday, 9:00 AM-5:30 PM; Occasional weekends as needed.

Purpose Statement/Position Summary: The Health Education Associate I responsibilities include conducting outreach activities, identifying community resources, delivering individual and or group health education services, distributing outreach materials, directly contacting targeted community members. Responsible for data collection, as needed, for program evaluation.

The Adolescent Medicine HIV Program team is dedicated to supporting the health, wellness, and overall well-being of youth and young adults through comprehensive health education, care navigation, outreach, and resource coordination. Working within a collaborative and community-focused environment, the team partners with patients, families, and community organizations to identify health-related and social needs, provide education, and connect individuals to both hospital-based and community resources. Through initiatives such as the SAMHSA Headspace Project, the team plays an active role in promoting behavioral health, substance use prevention and treatment, HIV/STI prevention and testing, and access to supportive services. Staff work across multiple systems of care to help patients navigate complex healthcare and social service networks while fostering engagement, wellness, and stability. The program also facilitates group-based health education, prevention activities, and resource connections designed to empower youth and young adults to make informed decisions and achieve positive health outcomes.

Minimum Qualifications/Work Experience:

Required:

  • 1+ years of related experience.

  • Local travel to events required and must be able to lift and transport items to community events.

  • General knowledge of family systems, health and safety, and parent education.

  • Live scan required as applicable per county contract.

  • Obtain American Heart Association BLS certification within the first 2 months of employment as applicable per county contract.

Education/Licensure/Certification: College education preferred or equivalent combination of experience and education.

Pay Scale Information

$47,840.00-$51,376.00

Final determination of a successful candidate’s starting pay will vary based on a number of factors, including, but not limited to, education and experience within the job or the industry. The pay scale listed for this position is generally for candidates who meet the specified qualifications and requirements listed on this specific job description. Additional pay may be determined for those candidates who exceed these specified qualifications and requirements.
