Job Summary:
The Patient Navigator-Orthopedics is an experienced clinical professional who provides guidance and education to patients and families, as well as a variety of direct and indirect patient care activities for an assigned provider and orthopedic service (shoulder, hand, spine, joints, non-surgical, sports medicine, pediatrics, podiatry). This position is responsible for assisting the patient throughout the duration of care and serves as the main contact for the patient. The Patient Navigator-Orthopedics has the significant responsibility of and accountability for managing the service for the provider.
Core Responsibilities and Essential Functions:
Coordination of Care: Helps patient coordinate appointments across multidisciplinary care teams and refers them to internal/external resources as needed. Acts as a liaison between patient, nurses, physicians, allied health professionals, and others on the care team. Collaborates with team to develop a comprehensive care plan based on the patients individual needs and preferences. Patient Advocacy: Serves as the primary educational resource for patients and families on care provided. Communicates relevant information to patients, family-member, physicians, and members of the patient care team. Addresses patient concerns and questions they might have concerning care. Provides education strategies to encourage a sense of empowerment. Patient Advocacy: Serves as the primary educational resource for patients and families on care provided. Performs clinical duties to include, but not limited to obtaining vital signs, administering limited profile of medications/injections per approved policy, performing simple diagnostic tests, collecting specimens, drawing blood, cleaning equipment, ordering/stocking supplies, and maintaining exam rooms. Must understand the basics of proper fitting and bracing and may be trained to apply DME. Coordinates with surgical team to ensure appropriate sets and pre-op plans are communicated, discusses post operative expectations at pre-op visit, organizes pre-op patient information packets for surgery, coordinates with the tissue bank to assure correct grafts/biologics for surgical cases. Enters orders for provider to co-sign, documents patient discussions in the medical record, completes FMLA/Short Term Disability/home bound paperwork. Performs other duties as assigned Complies with all Wellstar Health System policies, standards of work, and code of conduct.
Required Minimum Education:
- Accredited Program Medical Assisting
Required Minimum License(s) and Certification(s):
All certifications are required upon hire unless otherwise stated.• BLS-I - Basic Life Support - Instructor or BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support
- CMA (AAMA) - Cert Medical Assistant or RMA (AMT) - Registered Medical Assistant or CCMA (NHA) - Certified Clinical Medical Assistant or NCMA (NCCT) - National Certified Medical Assistant or NRCMA (NAHP) - Nationally Registered Certified Medical Assistant
Additional License(s) and Certification(s):
Required Minimum Experience:
