JOB DESCRIPTION:Home Infusion Nurse Coordinator- Home Infusion Nurse (In Patient Home)

The UVMMC Home Infusion Nurse Coordinator (HINC) is responsible for assuring appropriate transition of patients to the home infusion service by creating, revising and assessing the patient plan of care relative to identified criteria. The Home Infusion Nurse Coordinator teaches patients, care givers and home health care nurses to administer infusion therapy in the home and makes recommendations regarding safety to MD and inpatient Case manager. The HINC will coordinate with health care team members to assure timely progression of patients across the continuum of care and coordinates with home health care professionals to assure all involved are prepared to participate in home infusion therapy for the patient. The HINC maintains contact with patient, care givers and Health Care Professionals through assessments via phone weekly or as needed. HINC supports quality improvement through evaluation of variances to identified protocols and maintains quality data systems, event reports and identifies and participates in quality projects and maintains inventory particular to patient needs.

TheHome Infusion Nurse Coordinator – Home Infusion Nurse (In Patient Home)provides service in the home. The home infusion nurse (in patient home) delivers full range of nursing care to include assessing patient, starting and administering infusion, de-access and documentation of care. This may include the insertion of an intravenous catheter or accessing a central line for administering the medication.

EDUCATION:

State of Vermont Registered Nurse (RN) license required. The University of Vermont Medical Center is moving toward an all Bachelor of Science in Nursing (BSN) workforce. All external candidates hired into, or current employees promoted to, a RN position will be required to obtain their BSN as outlined in the terms of their offer letter. Current internal RNs are strongly encouraged to pursue a BSN to support this initiative.

EXPERIENCE:

Two to five years of related clinical experience such as case management, home or public health nursing experience, or a combination of education and experience from which comparable knowledge and skills are acquired. For home infusion nurse (in patient home) position, infusion and vascular access experience required/strongly encouraged.
