Purpose:
Is responsible for assessing new clients and reassessing existing clients. May be asked to participate in the packaging of congregate and home delivered meals as necessary.
Responsibilities:

Use an Area Agency on Aging prescribed form to assess potential clients as assigned by the Prime-Time Office Staff

Use an Area Agency on Aging prescribed form to reassess clients as assigned by the Prime-Time Office Staff

Notify the Director or Prime Time Office Staff of any client found to be at high risk or of a change in any client's status

Assist Prime Time Office Staff in the preparation of client files

Package food for distribution by home delivered mean staff

Other duties as assigned

High school diploma or equivalent.
Licensure, Certifications, and Clearances:
- Driver's License
