Health Care Associate | PRN, PT, FT

The Role The Health Care Associate provides compassionate, hands-on personal care and support to participants at the Williams Adult Day Center. Working closely with the RN Care Coordinator and other members of the Williams Adult Day Center team, this position helps ensure participants are safe, comfortable, engaged, and supported throughout the day. What Youll Do - Provide personal care according to each participants plan of care and direction from the RN Care Coordinator or designee. {=html} - Assist participants with transfers, ambulation, activities, and programs. {=html} - Observe participants and promptly report unusual occurrences or changes in condition. {=html} - Assist with obtaining vital signs and weights as directed. {=html} - Maintain accurate and timely participant records. {=html} - Follow established procedures for participant medical emergencies. {=html} - Build positive, respectful relationships with participants, families, and team members. {=html} - Participate in required meetings, in-services, and training. What Were Looking For - High school diploma or equivalent. {=html} - Certified Nursing Assistant (CNA) or Personal Care Assistant (PCA) certification or at least one year of related experience in geriatrics or caring for individuals with dementia. {=html} - Current CPR and First Aid training. {=html} - Experience working with older adults preferred. {=html} - Good judgment, communication, interpersonal, and recordkeeping skills. {=html} - Ability to assist participants with transfers and mobility. {=html} - Ability to lift 25 pounds or more and stand and walk for most of the workday.
