Full job description

Schedule: Monday - Friday 9am to 5pm | Job Location Type: In Person

Rehabilitation and recovery services are designed for individuals living in the community who live with serious mental illness. Our team helps individuals obtain housing, employment, mental health services, and more.

What to expect.

This is a direct care opportunity to exercise your independent judgement and assess and enroll clients in the appropriate services when they first enter into the rehabilitation and recovery program(s). Your efforts will be a part of ensuring a successful transition for each client.

Specific responsibilities include:

  • Educating clients and families/support systems about available services.

  • Providing linkages to outside providers as needed.

  • Performing administrative tasks related to internal and external enrollment requirements

  • Providing transportation for clients from state hospitals to the Rehabilitation and Recovery programs.

This position has a flat pay rate of $21.37/hr.

What we need from you.

  • A high school diploma or GED, bachelor’s degree preferred.

  • Minimum of 1 year positively referenced related work experience.

  • Knowledge of best practices in the areas of psychiatric rehabilitation and crisis intervention.

  • Knowledge and experience in the areas of assessment, treatment planning and interventions for persons with serious mental illness to include co-occurring disorders.

  • Basic knowledge of social security benefits and medical assistance.

  • A driver’s license with 3-points or less and access to an insured vehicle.
