How youll make an impact in this role - Provide direct nursing care to long-term care residents in accordance with established clinical standards. - Implement and monitor individualized care plans, documenting and communicating changes in condition. - Coordinate with physicians and interdisciplinary teams to carry out orders and ensure seamless care. - Promote resident safety and support effective discharge planning.
