RN Home Care Supervisor - Part Time with Benefits 9am-2pm

Liberty Home Care is looking for a motivated Registered Nurse to join our Licensed Home Care team as an RN Field Supervisor! This Part Time opportunity offers great benefits and a consistent M-F 9a-2p - 25 hour per week schedule.

Schedule: 9am - 2pm Monday - Friday (25 hours per week)

Duties & Responsibilities:

• Provide assessment of newly referred patients and assess health needs in home care setting

• Work with patient to create a patient centered plan of care

• Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family

• Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services

• Provide supervision and training of PCA/HHA staff

Qualifications:

• NYS Licensed Registered Professional Nurse

• Minimum of Associate's Degree required

• One year Nursing experience required

• Home care experience a plus
