Physical Therapist (PT) / Occupational Therapist (OT) - PRN Home Health Visits | Nokomis, Port Charlotte, Punta Gorda, F

Matrix Home Care is seeking experienced and compassionate Physical Therapists (PTs) and Occupational Therapists (OTs) for PRN home health visits across multiple Florida service areas. This position is ideal for clinicians who enjoy providing individualized therapy services in the patients home and are comfortable working independently while collaborating with the interdisciplinary care team. Open service areas include: Physical Therapist (PT): - Nokomis - Lehigh Acres - Arcadia Occupational Therapist (OT): - Port Charlotte - Punta Gorda North - Punta Gorda South - Fort Myers - Cape Coral - Lehigh Acres - Arcadia Responsibilities: - Complete therapy evaluations and follow-up visits in the patients home - Assess patient needs, functional limitations, mobility, safety, strength, coordination, activities of daily living, and home environment needs - Develop individualized treatment plans and goals based on patient condition and physician orders - Provide skilled therapy interventions to improve safety, mobility, independence, functional performance, and quality of life - Educate patients and caregivers on home exercise programs, adaptive techniques, transfer safety, fall prevention, equipment use, and home safety - Monitor patient progress and update treatment plans as appropriate - Communicate changes in condition, progress, and recommendations to the care team and physician as needed - Complete accurate and timely documentation in accordance with home health standards and regulatory requirements - Maintain compliance with agency policies, AHCA requirements, and applicable Florida home health regulations Requirements: - Active Florida Physical Therapist or Occupational Therapist license - Prior home health experience preferred - Current CPR/BLS certification preferred - Valid drivers license, reliable transportation, and auto insurance - Strong assessment, documentation, time management, and communication skills - Ability to work independently in the home setting - Comfortable traveling within assigned service areas Benefits: - PRN / flexible scheduling - Competitive per-visit rates - Weekly pay - Direct deposit - Supportive clinical and administrative team - Opportunity to accept visits based on your availability - One-on-one patient care in the home setting - Opportunity to grow with Matrix Home Care Position Type: PRN / Per Diem Schedule: Flexible, based on patient visit needs Service Areas: Nokomis, Port Charlotte, Punta Gorda, Fort Myers, Cape Coral, Lehigh Acres, and Arcadia, FL Pay: Competitive per-visit rates Background Screening Requirement: Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
