Behavioral Health Care Manager

• Schedule: Full-time | Monday-Friday 8am-4:30pm

How you’ll make an impact in this role

• Support Post-Discharge Continuity: Guide Medicare patients through their post-discharge care plans, ensuring they successfully navigate next steps and stay connected to essential follow-up services.

• Conduct Critical Clinical Evaluations: Perform vital mental health screenings and risk assessments, including PHQ-9, GAD-7, and Columbia Suicide Severity Rating Scales (C-SSRS).

• Facilitate Mental Health Connections: Serve as a bridge to mental health resources by efficiently matching and connecting patients with appropriate therapeutic services.

• Optimize Specialized Care Pathways: Direct patients to the correct interventionalists and specialists, ensuring they receive timely, targeted medical and behavioral interventions.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider preferred. American Heart Association or American Red Cross accepted.

• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

• Counselor preferred.

• Social Worker preferred.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Additional Preferences:

• Personalized behavioral health coaching and crisis intervention experience a plus

• Degree in Social Work or Psychology a plus
