St. Luke Hospice LPN- Monticello 8124-LPN-Hrly-7221 PT A Hospice LPN provides compassionate, endlife nursing care. This role focuses on comfort, symptom management, and emotional support for patients and families. Key Responsibilities Deliver bedside nursing care, including medications, wound care, and symptom relief Monitor patient condition and report changes to the RN/Case Manager Provide education and emotional support to patients and families Document care accurately and follow hospice care plans Work collaboratively with the interdisciplinary hospice team Qualifications Current LPN license Strong communication and compassion Experience in hospice or home health preferred
