Full job description

Love nursing but ready to leave 12 hour shifts behind?

At Paragon Home Care, we help seniors live safely and comfortably in their own homes. We're looking for an experienced Registered Nurse to serve as our Clinical Manager, a new leadership role built around what nurses tell us they miss most: time to actually teach, assess, and make a difference, without the pressure of a hospital floor. You will also be designated as our Director of Nursing for Virginia home care licensing.

Our team specializes in dementia care, and our office and care management staff are Certified Dementia Practitioners. We'll sponsor your certification, too!

You'll oversee the plan of care for every client we serve, visit clients in their homes to assess their needs and safety, and coach our caregivers one on one so they can deliver excellent care every day. You'll also lead the clinical side of our caregiver training program. Your work will directly raise the quality of care for every family we serve. Your week will be split between our McLean office and visits to client homes. This is not a remote position.

What you'll do

  • Complete the nursing assessment and write the plan of care for every new client, in partnership with the client and family
  • Review every client's plan of care, and supervise services at least every 90 days or sooner when a client's condition changes
  • Evaluate each client's home setup and safety equipment, and identify fall risks and gaps
  • Build trusting relationships with clients and their families through a consistent, personal approach, supporting each client's physical, emotional, social, and spiritual wellbeing
  • Train caregivers on site in the equipment and techniques each client needs, including transfers, mobility aids, bathing and toileting equipment, skin care, positioning, and adaptive ADL tools
  • Coach caregivers to build their skills and confidence
  • Validate caregiver skills through hands-on competency check-offs at hire and annually
  • Build strong, lasting connections with our caregivers as a trusted mentor and advocate, supporting their growth, confidence, and sense of purpose in the work they do
  • Own the clinical content of our caregiver training program, starting with dementia care, and lead infection control training
  • Document assessments and training sessions accurately in our care management system
  • Recognize when a client needs medical follow up, and coordinate with families, physicians, and care managers
  • Partner with our Client Services team on client satisfaction, quality of care, and resolving client concerns
  • Partner with our HR team to recruit, screen, orient, and train caregivers
  • Represent Paragon's clinical expertise to hospitals, physicians, care managers, and senior living partners, alongside our Community Engagement team
  • Serve as our designated Director of Nursing for Virginia licensing compliance, the registered nurse responsible for supervising all personal care services. Keep our policies, records, and practices survey-ready, and serve on our quality improvement committee

What we offer

  • $88,000 to $95,000 per year, based on experience
  • $3,000 sign-on bonus
  • Monday- Friday, business hours, with a limited, shared after-hours on-call rotation. Occasional Saturdays are flexible, and when you work one, you take a weekday off
  • Company car provided for all client visits, so you never put miles or wear and tear on your own vehicle
  • Medical, dental, and vision insurance, eligible upon hire
  • Paid time off, including vacation, personal, and sick days, plus paid holidays
  • 401(k) with company match
  • Aflac supplemental insurance options
  • A structured orientation working directly with our leadership team
  • Flexible start date, so you can give proper notice to your current employer
  • A seat on our leadership team, reporting directly to our COO
  • Paid Certified Dementia Practitioner (CDP) certification and continuing education support

What you bring

  • Compassion and genuine care for seniors, their families, and our caregivers, and a desire to grow with the Paragon family and culture
  • Active Virginia RN license or multistate compact RN license
  • ADN or BSN
  • At least 3 years of RN experience. Home health, hospice, geriatrics, rehab, assisted living, or long term care experience is strongly preferred. If you're coming from a hospital, we'll help you make the transition.
  • At least 1 year supervising, teaching, or managing CNAs, aides, or nursing staff
  • Dementia care experience; CDP certification is a plus
  • Commitment to client privacy and HIPAA standards
  • Valid driver's license and a clean driving record
  • Ability to pass a criminal background check and TB screening
  • Ability to demonstrate transfers and assist with lifting up to 50 lbs
  • Ability to work on site at our McLean office and in client homes. This is not a remote role, and regular presence in the office is required.
  • Warm, patient communication style and a genuine love of teaching
