Full job description

Nurse Educator- RN The qualified candidate will join the Envision Wellness interdisciplinary team and provide evidence-based diabetes self-management education and support (DSMES), nursing assessment, chronic disease education, care coordination, and population health services. Responsibilities include developing individualized care plans; providing diabetes education; supporting CGM, insulin pumps, and other diabetes technologies; coordinating care with providers and other healthcare professionals; conducting patient outreach and follow-up; and participating in community wellness, health screenings, quality improvement, and program development. Candidate must provide excellent patient care, communicate effectively, work independently and collaboratively, and have strong computer/EMR skills.
