Overview
Value-based Advance Practice Provider managing chronic conditions of high-risk geriatric panel Monday-Friday, no weekends or evenings. Use Athena EMR + Marvix AI Scribe to get charting done during the workday. Start day around 8AM - end around 4:30/5PM.

Responsibilities
The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The APPs who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
Qualifications

Licensed as a NP/ PA in the State where HarmonyCares Medical Group office located

Federal DEA License/eligibility
-
Geriatric training/experience preferred

Active CPR/BLS Certification

Board Certification
-
Valid and unencumbered driver's license with good driving record
