Nurse Practitioner or Physician Assistant (Memory Care)

Job Duties

We are seeking a Physician Assistant or Nurse Practitioner to join the M Physician's Memory Care team. This is an outpatient role and will be practiced at the M Physician's St. Louis Park location.

Practice Details:

Conduct comprehensive cognitive assessments, including history-taking, neurological exams, and validated screening tools.

Develop and adjust individualized care plans in collaboration with your Memory Care neurologists and others on the treatment team.

Provide education and counseling to patients and caregivers on disease progression, treatment options, and community resources.

Respond to urgent or emergent neurological issues related to prescribed treatments, such as anti-amyloid therapies, to evaluate and triage next steps.

Participate in interdisciplinary case conferences and contribute to quality improvement initiatives in memory care.

Ongoing collaboration with all members of our care team, patients and families

Additional Practice Details:

Full time 40 hours per week

Monday - Friday

No current weekend or holiday requirements

Position Qualifications:

A qualified candidate will be a graduate of an accredited school of nursing and formal Nurse Practitioner training program or a graduate of an accredited Physician Assistant program. Have a current MN RN license and be approved or eligible by the Minnesota Board of Nursing for prescription writing privileges or a current PA license with prescriptive authority through the MN Board of Medical Practice. Be certified through ANA / AANC or eligible for ANA/AANC national exam or through the NCCPA and meet threshold criteria/qualifications for credentialing and privileges.

Preferred Qualifications:

3 years as an advanced practice provider experience in a neurologic care setting is preferred. New graduates with an interest in neurology are encouraged to apply.
