Job Summary:
The STRIVE Therapist provides trauma-informed, developmentally appropriate clinical services to young adults ages 18–21 who may be involved with the Division of Child Protection and Permanency and are preparing for greater independence. The therapist works collaboratively with clients, families, program staff, DCPP, and community providers to support behavioral health, stability, and successful transition into adulthood. The Expecting Parenting Youth Program offers the same services to young parents who have aged out or transitioning to age out of DCPP.
What You Will Do:

Complete comprehensive clinical assessments at intake and throughout treatment to identify behavioral health needs, strengths, risk factors, trauma history, support systems, and barriers to successful independent living.

Provide individual and family therapy, crisis intervention, safety planning, and other clinical supports using trauma-informed, strengths-based, and developmentally appropriate approaches.

Function as an active member of the interdisciplinary STRIVE/EPY team, collaborating with case managers, program leadership, DCPP representatives, healthcare providers, schools, and community partners to coordinate comprehensive care.

Provide clinical leadership and consultation within the team by identifying emerging concerns, supporting intervention planning, monitoring treatment progress, and helping guide responses to client needs and risk.

Develop individualized psychosocial treatment plans with meaningful, behaviorally measurable goals and objectives that reflect each client’s clinical needs, personal goals, and transition to adulthood.

Plan, coordinate, and communicate discharge, transfer, and aftercare services to promote continuity of care and connect clients with appropriate behavioral health, medical, parenting, housing, educational, and community-based supports.

Maintain timely and accurate clinical documentation, including assessments, treatment plans, progress notes, crisis documentation, treatment reviews, discharge summaries, and service outcomes in accordance with agency and regulatory requirements.
What Makes You an Ideal Candidate:
-
Master's degree in a behavioral health discipline from an accredited college or university

1-2 years of clinical experience in the mental health field, including but not limited to internships, community-based services, case management, etc
-
Must have the ability to respond to clients for crisis intervention.

Provisional/Professional Licensure to practice counseling/clinical social work (LAC, LSW, LPC, LCSW, LMFT)

A valid driver's license and you must be insurable by the agency's automobile insurance
