Overview
HYBRID Case Management Role in Infusion Center at Piedmont Rockdale!
Responsibilities Collaborates with various medical providers and clinical teams to provide patient/family advocacy, assessment, and psychosocial interventions for high risk medical and surgical populations in the outpatient setting. Operates autonomously within the social work discipline to follow through and manage referrals to resolution. The Social Worker provides coping support and appropriate concrete services, strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources. Qualifications
Education
- Master’s degree in Social Work (MSW) from a program accredited by the Council on Social Work Education Required
Work Experience

Previous oncology experience. Previous Medical experience (hospital, nursing home, home health care, or dialysis) Preferred
Licenses and Certifications
- LMSW - Licensed Medical Social Worker - State Licensure LMSW in state of Georgia or persons who have obtained a master's degree from a program accredited by the Council on Social Work Education can practice social work under direction and supervision while preparing to take the master's social work licensing examination for a period of up to one year following the granting of such degree Upon Hire Required
