BETTER TOGETHER POSITION SUMMARY The Digital Care Advanced Practice Provider provides structured support for asynchronous patient care and patient portal management across primary care. This role focuses on managing defined categories of clinical work that can be safely addressed outside of traditional primary care provider workflows, while maintaining appropriate escalation and continuity. While the role is designed to be performed primarily remotely, limited in-clinic support may be provided in exceptional circumstances with appropriate compensation provided if in person work is required. The position works under the general direction, guidance, and supervision of the assigned Clinic Medical Director, with additional accountabilities to the collaborating Clinic Medical Directors, Senior Medical Director and the Chief Medical Officer. The Benefits of Working at GHC-SCW. JOB SPECIFIC MINIMUM QUALIFICATIONS Physician Assistant: 1. Bachelor of Science degree in an accredited Physician Assistant program required. 2. Physician Assistant Certification by the State of Wisconsin under the rules of the Medical Examining Board required. 3. Current (or ability to attain) membership in professional Physician Assistant organization(s) required. Nurse Practitioner: 1. Master's degree from an accredited college or university in Nursing with specialty preparation as a Nurse Practitioner. Certification complementary to primary care practice area is required. 2. Valid State of Wisconsin license to practice as a Registered Nurse and as a Nurse Practitioner required. Advanced Practice Nurse Prescriber certification is desirable but not required. Specialty: 1. Primary care practice experience as an NP or PA is strongly preferred. 2. Experience with Epic electronic health records, including in-basket management, is strongly preferred. 3. Comfort with remote/telehealth work and asynchronous clinical communication is strongly preferred. CORE 1. Possession of or eligibility for a valid Healthcare Provider CPR/AED certificate is required. Employee must obtain certification within four (4) months of employment 2.
