Location: Remote* (North Carolina residents preferred)
Pay Rate: $40-43/hr
Employment Type: Contingent with potential pathway for contract extension or conversion
Schedule: Monday-Friday, 8:00 AM-5:00 PM EST
Why This Role Matters
As an RN Case Manager, you will serve as a critical advocate for members by coordinating care, identifying barriers to treatment, and supporting positive health outcomes. Through comprehensive assessments, care planning, and ongoing member engagement, you'll help ensure members receive the right care at the right time.
What You'll Do

Conduct comprehensive clinical assessments to evaluate members' healthcare needs and support requirements

Develop and implement individualized, member-centered care plans based on health status, goals, and available resources

Coordinate care across multiple settings including hospitals, physician offices, rehabilitation facilities, skilled nursing facilities, and home care services

Perform telephonic outreach and follow-up with members, prioritizing recently discharged patients and high-risk cases

Monitor member progress, assess outcomes, and modify care plans as needed

Educate members regarding health conditions, treatment options, and available community resources

Identify barriers to care and facilitate appropriate referrals and interventions
-
Review treatment plans, assess benefit eligibility, and evaluate medical necessity when applicable

Maintain accurate and timely documentation of assessments, care plans, interventions, and outcomes

Collaborate with providers, family members, and internal partners to support quality care delivery
What We're Looking For

A current, up-to-date resume
-
Active RN license required (NC or Compact/Multi-State License)
-
Minimum of 3 years of clinical and/or case management experience
-
Hospital-based case management or clinical nursing experience required
-
Strong telephonic communication and customer service skills
-
Excellent time management, organizational, and prioritization abilities
-
Ability to manage a caseload in a fast-paced environment
-
Insurance case management experience preferred
-
Medicare experience preferred
-
CCM (Certified Case Manager) certification preferred
-
Strong technical proficiency and ability to navigate multiple systems efficiently
-
Telephonic case management experience preferred

Ability to meet compliance requirements

Must be eligible and provide proof of eligibility to work in the US
