JOB REQUIREMENTS: Provides home care nursing in accordance with the physician's plan of care. Provides advice & support to the client's family. Reports significant changes in the client's condition to the family, RN Clinical Manager, & physician. Obtains physician orders for changing client care needs. Maintains accurate & current plan of care, physician's orders, and medication profile for the client.
