RESPONSIBILITIES: Patient Care. Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care; Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems Evaluation & Documentation. Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff REQUIRED QUALIFICATIONS: Nursing Diploma required License and current registration to practice as Registered Professional Nurse in New York State Current NYS Driver's license PREFERRED QUALIFICATIONS: 6 months- 1 year experience conducting UAS-NY Community Assessment preferred. Bachelors degree of Science in Nursing preferred. 1 year of experience working with a frail or elderly population preferred. Availability of Motor Vehicle EDUCATION: AS: Nursing (Required) PHYSICAL REQUIREMENTS: Drive - Clinical - Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments. Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients' homes or other confined, unpredictable, or uncontrolled environments. These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery. Emotional and cognitive demands are substantial due to rapid decision-making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. Valid licensure, physical readiness, and flexibility for extended or on-call schedules are typically required. LICENSES/CERTIFICATIONS: RN - Registered Nurse - NYS Office of Professions BLS - Basic Life Support - American Heart Association (AHA) PAY RANGE: $75,733.84 - $101,014.16 The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
