Job Summary
Student Health & Wellness (SH&W) at the University of Utah welcomes applications for Student Health & Wellness Case Manager. The position is an integral part of the SAMHSA Garrett Lee Smith ( GLS ) Suicide Prevention Grant, titled CREST : Coordinated Referrals, Evidence-based Support, and Training, awarded to the University of Utah. CREST is a strategic capacity-building initiative focused on the reduction of suicide and substance use disorder on campus amid growing demand. Our coordinated approach will ensure students receive appropriate care while equipping the campus community to recognize and respond to students in crisis. This position reports to the Associate Vice President of Student Health & Wellness and works closely with SH&W and Student Affairs departments. It is anticipated that this position will continue at the conclusion of the grant.

Responsibilities
-
Serve as SH&W’s central point of contact for students seeking care coordination across internal departments, e.g., University Counseling Center ( UCC ), Center for Campus Wellness ( CCW ), Student Health Center ( SHC ) and external providers.
-
Design the division’s case management model from the ground up, including scope of service, eligibility criteria, intake and referral pathways, caseload parameters, documentation standards, escalation thresholds, and outcome measures.
-
Writes the standard operating procedures governing the program where none currently exist, and revises them as practice reveals what the initial design missed.
-
Determines priorities, sequencing, and method independently against division goals, with consultation available but not required for day-to-day decisions.
-
Identifies service gaps the division has not yet named and proposes solutions before being asked to.
-
Provide follow up case management for students referred for care outside the UCC , such as intensive outpatient, inpatient, residential, detoxification, or rehabilitation services.
-
Facilitate warm hand-offs and referrals to both internal SH&W departments and external community providers, ensuring students do not disengage after referral.
-
Help create, negotiate, and maintain formal MOUs with external behavioral health providers to establish clear referral and care pathways.
-
Provide consistent, proactive follow-up with all referred students to confirm they engaged with care and to help remove barriers to continued treatment.
-
Initiate contact with students returning to campus following mental health/substance use disorder-related hospitalization, serving as a single-entry point to reconnect them with on-campus services.

Liaise with the Behavioral Intervention Team ( BIT ), Office of the Dean of Students ( ODOS ), University Counseling Center ( UCC ), and other key campus partners in determining appropriate levels of care for high-acuity or complex/co-occurring cases.
-
Maintain a caseload of ~60 students with more complex care coordination needs; refer students needing only internal, lower-acuity coordination to Peer Well-Being Navigators.
-
Use data and direct experience to help inform care coordination policy, standards of practice.
-
Track referral, engagement, and outcome data for institutional and federal grant reporting.
-
Provide outreach to the campus community on available care coordination services.
-
Other relevant duties as assigned.
Skills and Knowledge Required
-
Strong understanding of care coordination and case management best practices in a behavioral health setting.
-
Demonstrated experience building a program, service, or function that did not previously exist. Candidates should be prepared to describe specifically what they inherited, what they created, and how they determined it was working.
-
Demonstrated ability to write standard operating procedures governing the program where none currently exist and revises them as practice reveals what the initial design missed.
-
Demonstrated experience operating effectively in environments where procedures were incomplete, absent, or in development.
-
Evidence of sound independent judgment in complex situations involving multiple stakeholders and competing obligations.
-
Establishing program data, reporting structure, and accountable for demonstrating impact.
-
Demonstrated ability to build and sustain professional relationships and formal agreements with external healthcare and treatment providers.
-
Knowledge of community mental health/substance use disorder treatment resources, including different levels of care.
-
Strong interpersonal, communication, and self-advocacy coaching skills, with the ability to support students navigating complex health systems and insurance.
-
Ability to work independently across multiple internal and external stakeholders.
-
Skilled in maintaining accurate case records and tracking data for reporting purposes.
-
Sensitivity to confidentiality requirements ( HIPAA / FERPA ).
-
Ability to develop and maintain current knowledge of on-/off-campus behavioral health resources.
Minimum Qualifications
-
Bachelor’s degree in Health Care Administration, Business, Communication, Nursing or related field, or equivalency (one year of education can be substituted for one years of related work experience).
-
Demonstrated human relation and effective communication skills are also required.
-
Experience in oncology, multidisciplinary clinics, and/or working with high-acuity, complex patients may be preferred.
This position is patient-sensitive and must fulfill all associated requirements. Bachelor’s in Social Work, Psychology, similar; Master’s preferred.

4-6 years experience providing case management or care coordination, particularly with high-acuity or complex/co-occurring cases, strongly preferred.

Experience working directly with college students preferred.

Familiarity with local community resources and systems a plus.
