Your future role at a glance

  • Department: Adult Inpatient Unit

  • Schedule: Day shift | Full-time

How you’ll make an impact in this role

  • Create a plan for care across the continuum, integrating patient/family preferences and values.

  • Monitor patient care through assessments, evaluations and/or patient records.

  • Advocate for resources and removal of barriers.

  • Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

What minimum requirements you’ll need

Licensure / Certification / Registration:

  • One or more of the following required:

  • Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

  • Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

  • Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

  • Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job-specific experience required.

Additional Preferences:

  • Master's Degree in Behavioral Science, Social Science, or Psychology required

  • LPC/LSW licensure required

  • Behavioral Health experience preferred
