BRING YOUR SKILLS TO YOUR PATIENTS' FRONT DOOR.

Home Health Physical Therapy is where you can see the real impact of your work. You'll treat patients in the place they actually live, work with their families, and build the kind of long-term relationships that clinic settings rarely allow. If you want autonomy, variety, and time to focus on the whole person, this role is for you.

Nursing & Rehab At Home is a clinician-owned and operated, family-run agency, proudly rooted in our local community.

Many of our employees have been working with us for over a decade! We offer a relaxed, nurturing environment where patients and staff are both well cared for.

WHY YOU'LL LOVE IT HERE

• Real relationships. You'll get to know your patients and their families, and you'll see them get stronger, more confident, and more independent.

• Autonomy you can feel. You'll plan and guide care with trust and support from your Clinical Manager and team.

• A team that has your back. You're out in the community, but you're never on your own. With regular huddles, peer support, and celebrations, working with Nursing & Rehab At Home gives you colleagues who feel like family.

• Variety. No two days look alike. Different neighborhoods, different stories, different challenges.

  • Impact: You’ll help patients relearn daily activities, adapt their homes, and gain confidence, with results you can see.

• Collaboration: You’ll work closely with physicians, nurses, therapy assistants, and other clinicians as part of a coordinated care team.

WHAT YOU'LL DO

  • Evaluate patients’ functional abilities and assess their homes for hazards or barriers to independent living

  • Build individualized treatment plans and goals, and share them with referring physicians and the care team

  • Teach or retrain patients in daily activities, and train them in adaptive equipment, prosthetics, and orthotics, including fabricating splints when needed

  • Take on case management for your assigned patients when requested, coordinating their care from start to discharge

  • Complete OASIS assessments as assigned

  • Educate patients, families, and caregivers so they feel equipped to support recovery

  • Supervise certified occupational therapy assistants and, when therapy is the only skilled service, home health aides

  • Connect patients and families with other home health services they may need

  • Join case conferences and in-services to keep care coordinated and your skills sharp

WHAT YOU BRING

  • Graduate of an accredited occupational therapy program

  • NBCOT certification

  • Active California OT license

  • Current CPR certification

  • Reliable, insured vehicle and a valid driver’s license

  • Strong communication and organizational skills

  • 1+ year of OT experience preferred
