HomeWell Care Services | Glenview, IL Full time | $50,000 to $60,000 annual base compensation Experienced CNAs encouraged to apply. RN credentials a plus. Preferred start: ASAP or November 1
You know what great care looks like. Help us shape how families experience it.
HomeWell Care Services is a rapidly growing, locally owned home care agency seeking a personable, motivated professional ready to grow into care management. You will guide families, develop personalized care, and work directly with company leadership every day, with a voice in how we grow.
Based in Glenview, we serve Arlington Heights, Northbrook, Evanston, and surrounding communities. Our approach combines thoughtful caregiver matching, ongoing client relationships, and signature programs supporting independence, safety, and meaningful engagement.
What you will do
- Connect with prospective clients. Conduct phone conversations and assessments in clients’ homes. Build trust, understand their needs, and recommend appropriate support.
- Help families move forward. Explain HomeWell’s services, value, and pricing. Answer questions, address concerns, and confidently guide families toward starting care.
- Create and manage care plans. Develop clear caregiver instructions, maintain them in WellSky, and update them as clients’ needs change.
- Make starting care seamless. Prepare and explain agreements and required paperwork, secure signatures before care begins, and partner with our Caregiver Liaison to match clients with the right caregivers.
- Stay involved. Visit clients regularly, generally monthly, to gather feedback, review care, coach caregivers, and respond to changing needs.
- Support the team. Share on call responsibilities, serving as the primary contact for urgent matters from time to time, and occasionally provide caregiver coverage.
Who will thrive here
You are someone families trust and colleagues rely on. You communicate confidently, notice details, follow through, and take initiative. You welcome coaching and enjoy solving problems.
We are looking for:
- At least two years of relevant experience in home care, a healthcare facility, or a related setting, including assistance with daily living.
- Strong relationship building, communication, and writing skills.
- Comfort discussing service options and costs and helping families make decisions.
- Organization, sound judgment, and confidence using email, electronic forms, and care documentation software. WellSky experience is helpful.
- Reliable transportation, a valid driver’s license, current auto insurance, and availability to visit clients throughout our service area.
- Ability to meet applicable background screening and employment requirements.
RN credentials are preferred but not required. Experience as a CNA, lead caregiver, mentor, or care coordinator is especially relevant.
What we offer
- $50,000 to $60,000 annual base compensation, depending on experience and qualifications.
- Potential for performance based bonuses, depending on qualifications and the agreed compensation package.
- 401(k) and paid leave.
- Potential financial support toward individual health coverage, with details discussed during interviews as part of the compensation package.
- Meaningful responsibility and growth opportunities, working directly with company leaders.
- Training and professional development through HomeWell resources and practical experience.
Schedule
Full time, combining client visits, care coordination, and administrative work. Evening and weekend work may be required as needs arise, including assessments and shared on call coverage.
