Full job description

Provides comprehensive social work services to patients, families, and caregivers across a variety of clinical settings.

Conducts psychosocial assessments, develops and implements intervention and treatment plans, facilitates access to resources and services, and collaborates with interdisciplinary teams to support optimal patient outcomes.

Provides counseling, crisis intervention, care coordination, discharge and

transition planning, patient advocacy, and resource management services consistent with professional standards of practice, organizational

policies, and regulatory requirements.

Responsibilities may vary based on assigned department, specialty area, or clinical setting, including but not limited to inpatient, outpatient, behavioral health, psychiatry, emergency, specialty care, and care management services.

Job Description:

Essential Duties and Responsibilities

Clinical Assessment and Intervention

Conduct comprehensive psychosocial assessments to identify patient, family, behavioral health, environmental, social, and resource needs.

Develop, implement, and monitor individualized care, treatment, and intervention plans.

Provide counseling, supportive therapy, crisis intervention, and emotional support to patients and families.

Assess patient safety concerns and facilitate appropriate interventions and referrals.

Evaluate barriers to care and recommend strategies to improve treatment adherence and health outcomes.

Care Coordination and Case Management

Coordinate services across the continuum of care to ensure timely access to treatment, community resources, and support services.

Facilitate discharge planning, transition planning, and continuity of care activities as appropriate.

Collaborate with internal and external providers, agencies, facilities, and community organizations to address patient needs.

Assist patients and families in navigating healthcare systems, insurance benefits, financial assistance programs, and

community resources.

Advocate for patient rights, self-determination, and equitable access to services.

Behavioral Health and Therapeutic Services

Provide individual, family, group, and/or supportive counseling services as appropriate to the clinical setting.

Support patients experiencing behavioral health, substance use, emotional, psychosocial, or adjustment-related concerns.

Participate in treatment planning, case consultation, and multidisciplinary care conferences.

Facilitate referrals for specialized behavioral health, substance use, and community-based services.

Interdisciplinary Collaboration

Collaborate with physicians, advanced practice providers, nurses, therapists, care managers, and other healthcare

professionals to develop and implement patient-centered care plans.

Participate in interdisciplinary rounds, case reviews, and care coordination meetings.

Serve as a social work resource for patients, families, and healthcare team members.

Documentation and Compliance

Maintain accurate, timely, and complete documentation in the electronic medical record.

Ensure compliance with professional standards, ethical guidelines, accreditation requirements, federal and state regulations,

and organizational policies.

Maintain confidentiality and safeguard patient information.

Professional Practice

Maintain required licensure and professional competencies.

Participate in quality improvement, performance improvement, educational, and departmental initiatives.

Serve as a preceptor, mentor, or resource to students and staff as appropriate.

Perform other duties as assigned.

Minimum Qualifications

Education

Master's Degree in Social Work (MSW) from an accredited program

Licensure

Current Massachusetts social work license required.

LCSW or LICSW required based on departmental assignment.

Experience

Relevant clinical social work experience in healthcare, behavioral health, community health, psychiatric, ambulatory, or acute

care settings preferred.

Knowledge, Skills, and Abilities

Knowledge of social work principles, practices, ethics, and standards of care.

Knowledge of behavioral health, psychosocial assessment, crisis intervention, and care coordination practices.

Understanding of healthcare systems, community resources, and patient advocacy principles.

Strong communication, counseling, conflict resolution, and interpersonal skills.

Ability to effectively manage complex patient situations and competing priorities.

Ability to work independently and collaboratively within interdisciplinary teams.

Proficiency with electronic medical record systems and standard computer applications.

Working Conditions

Work is performed in a variety of healthcare settings, including inpatient, outpatient, ambulatory, behavioral health, specialty, and community-based environments. May involve exposure to patients experiencing acute medical, behavioral health, psychosocial, or crisis related

conditions.

Pay Range:

$68,078.40 USD - $110,323.20 USD

The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
