Case Manager Supervisor

PROGRAM MANAGER Job Summary The Program Manager oversees program operations and staff to ensure effective service delivery, accurate documentation, compliance with program requirements, and progress toward client goals. This position provides direct client support while supervising staff, monitoring case management activities, maintaining data accuracy, and completing required reports. Essential Duties and Responsibilities - Lead and manage program staff through hiring, orientation, training, supervision, coaching, performance evaluations, and ongoing feedback. - Oversee case management operations, including accurate documentation, regular case plan reviews, and compliance with record-keeping standards. - Provide direct services and support to clients, including managing a caseload, making referrals, and advocating for access to appropriate community resources. - Collaborate with Case Managers, Crisis Prevention Coordinators, and Housing Specialists to develop and maintain current resource lists for health, mental health, substance use, education, and job training services. - Monitor ClientTrack and ServicePoint to ensure case notes, reports, client entries and exits, annual updates, and progress toward client goals are accurate and up to date. - Review database entries for accuracy and maintain data integrity and completeness. - Prepare and submit required reports accurately and on time in accordance with funder, agency, and program requirements. - Monitor program performance and address areas requiring improvement based on quarterly reports from the Mayors Office of Homeless Services (MOHS). - Work to ensure that at least 55% of clients served transition to permanent housing. - Work to increase employment opportunities for clients served by 20%. - Perform other duties as assigned. Required Driving Requirements - Driving an agency and/or personal vehicle is required. - Must be at least 21 years of age. - Must have at least two years of driving experience. - Must have a valid drivers license issued by the state of residence. - The drivers license must not have restrictions that would prevent the employee from operating a vehicle as required by the Agency. - Driving record must have no more than three (3) points.
