Continuing Care Coordinator

Overview

Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, theContinuing Care Coordinatorwill provide rehabilitation services to patients as a part of their overall treatment. TheContinuing Care Coordinatorwill work on a Full-Time basis out of Helio Health’sRochester Inpatient Program.

Pay Rate: $23.00 - $27.00 per hour.

In addition to Helio Health’s comprehensive benefits package, Full-Time hires for this position are eligible for aSign-On Incentiveup to$1,500!

Responsibilities

• Maintain a case load of patients seeking treatment from the effects of substance use and/or mental health disorders.

• Perform essential counseling responsibilities including screening, intake and orientation, assessment, evaluation and intervention, treatment planning, referrals, crisis intervention, counseling, patient education, case management, reporting and record keeping.

• Participate in program, administrative and clinical committees as assigned.

• Relate effectively with staff, clients, and community while upholding confidentiality requirements.

Qualifications

• Associate's Degree in a human service field with minimum of CASAC-T required.

• One year counseling experience preferred.

• Equivalent alternative combination of education and experience may be substituted for the educational requirements.

• Knowledge of addiction/recovery, behavioral health, self-help group process and principles of counseling.

• Proficiency with basic computer applications.
