The role!

Position: Adult Family Care Case Manager

Location: Wakefield, MA

Pay rate: $24.00 per hour

Hours: 20 hours per week (hours required to be worked in afternoon/evening)

Summary & Responsibilities

The Adult Family Care Case Manager provides case management for AFC Participants and Caregivers including development and evaluation of AFC Plan of Care, goal setting, training, advocacy, and referrals. This position requires the successful candidate to complete home visits in various locations (this includes Essex, Suffolk, and Middlesex county – locations up to 30 min drive from office) at various times based on family’s schedules. This often includes some early evening hours after 4pm.

  • Conducting the initial and ongoing assessments of the AFC qualified setting.

  • Obtaining a social history and conducting a psychosocial assessment and evaluation of the participant.

  • Assisting in selection, training, evaluation, and supervising of AFC caregivers in conjunction with the Director.

  • Conducting on-site visits with each participant and the caregiver in the qualified setting monthly or bi-monthly. Completing progress notes, ensuring the documentation is completed in a timely manner. Conducting periodic review of AFC care logs.

  • Assisting the participant in obtaining information and accessing other healthcare and community services.

  • Participating in ISP and team meetings as needed, attends other meetings as required.

Qualifications

  • Bachelor’s Degree in Social Work, Education, Human Services, or a related field is required.

  • At least two years of experience in the care of elders or those with disabilities.

  • A passion and dedication to supporting our people is a must

  • Valid Driver’s License and acceptable driving record
