Company OverviewPriority Cares Home Services is a Minority Business Enterprise (MBE) certified home healthcare agency providing waiver services under the New York State Department of Healths Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Medicaid waiver programs.We are committed to promoting independence, safety, and community integration for individuals with disabilities through person-centered, strengths-based care. ## Position OverviewWe are seeking a compassionate and skilled Master Social Worker (MSW) to provide Positive Behavioral Interventions and Supports (PBIS), Community Integration and Counseling (CIC) and Independent Living Skills Training (ILST) services under NYS waiver programs.The MSW will support individuals with disabilities and/or traumatic brain injuries by developing and implementing person-centered behavioral and independent living plans that promote positive behavior change, self-sufficiency, and community participation.This role requires strong clinical judgment, behavioral support skills, and the ability to work in home and community-based settings across New York City. ## Key Responsibilities### ILST (Independent Living Skills Training) Key Responsibilities * Provide skill-building support to help individuals live independently within the community * Assist participants with daily living skills, organization, scheduling, budgeting, and community integration * Support goal achievement outlined in the service plan * Maintain accurate documentation, progress notes, and communication with the care team * Encourage participant independence, safety, and self-advocacy### PBIS (Positive Behavioral Interventions and Supports) Key Responsibilities * Implement behavioral support strategies based on approved behavior support plans * Provide direct support to individuals with behavioral and emotional needs * Monitor behaviors, document progress, and report significant concerns to the clinical team * Promote positive coping skills, community participation, and person-centered support * Maintain compliance with NYSDOH and agency documentation requirements### CIC (Community Integration Counseling) Key Responsibilities * Support individuals with emotional adjustment and community reintegration * Assist participants in developing coping skills, social interaction, and independence * Provide person-centered counseling support aligned with service goals * Coordinate with interdisciplinary teams to support participant progress and well-being * Complete timely documentation and maintain confidentiality in accordance with NYSDOH standards ## Qualifications * Bachelors degree in Social Work, Psychology, or related human services field (MSW preferred) * Experience working with individuals with TBI, developmental disabilities, or behavioral challenges * Strong understanding of Positive Behavioral Interventions and Supports (PBIS) * Familiarity with NHTD and/or TBI waiver programs (preferred but not required) * Excellent communication skills * Strong documentation and organizational skills * Ability to work independently in community-based settings * Valid drivers license and reliable transportation required (preferred but not required) ## ## How to ApplyPlease send your resume and cover letter to: [email protected] Call/Text: (718) 400-6166 ext. 0Priority Cares Home Services is an Equal Opportunity Employer.
