Job Duties:• The CFTM Facilitator is responsible to provide training and coaching to CFTM Facilitators, County Child Welfare Services staff, and other internal DSS staff. The CFTM Coach is also responsible for presenting the CFTM process to external stakeholders and community partners. This role will be key in the implementation, maintenance, and monitoring of the agency wide CFTM process.

  • Provide CFTM trainings and certifications for all DSS staff as needed. Perform on-call duties and provide support for on-call CFTM facilitators.

  • Engage, train, and coach county office staff through in person office visits to identify areas of training, build facilitation skills, increase the quality of case manager led CFTM’s, and build county office capacity to independently conduct CFTM’s. Utilize data reports and spreadsheets to inform practice and guide improvements.

  • Conduct reviews and field observations for both facilitator and case manager led CFTM’s to improve quality and ensure fidelity of the CFTM model, and adherence to the program goals and objectives. Work collaboratively with CFTM Team Leaders to provide ongoing coaching and support for CFTM Certified Facilitator.

  • Engage and work collaboratively with internal and external stakeholders to promote CFTM practice and engage in problem solving of implementation. Provide CFTM trainings to external partners and providers as needed. Respond timely and professionally to constituent inquiries. Participate in regional and other meetings to promote CFTM practices and policies. Help ensure that CFT meetings address placement stability, permanency, crisis response, and transition planning.

  • Manage outcome reporting through collection of data and provide data reports as requested.

  • Ensures that Personal Identifying Information (PII) is properly used, accessed, gathered, shared and disposed. Protects the agency networks and applications by safeguarding systems, equipment, and data.

  • Other duties as assigned.

A Bachelor's Degree in Social Work, Behavioral Science or Social Science.

A Bachelor's Degree in any other related field and one (1) year of professional experience in human services or social service programs
