Organization: CPMC-California Pacific Med Center Position Overview: Responsible in assisting in the overall case management of designated patients in a particular program. Planning and implementing therapeutic interventions designed to promote optimal level of functioning and transition to a lower level of care. Job Description: EDUCATION:

SKILLS AND KNOWLEDGE:

Strong interpersonal skills and ability to get along with others.
Demonstrated leadership abilities.
Understanding of respective program goals/objectives
Strong ability to communicate in both oral and written form.

Pay Range is $37.32 to $55.99 / hour The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health’s comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
