Full job description

KeystoneCare is a nonprofit provider of in-home healthcare services in the Hospital & Health Care sector, supporting patients and families with home health, hospice, palliative medicine, and private duty homecare. Our work is centered on delivering thoughtful, personalized care where patients are most comfortable, helping people navigate recovery and ongoing health needs with dignity and support. As a Physical Therapist at KeystoneCare, you will make a meaningful impact by helping patients strengthen mobility, restore function, and maintain independence through compassionate in-home care. This role is ideal for someone who is energized by one-on-one patient support and collaborative care, and who wants to contribute to better outcomes for individuals and families across the communities we serve. Responsibilities

  • Provide personalized rehabilitation services to patients in their homes.
  • Assess, plan, and carry out treatment programs focused on improving mobility, strength, and overall functional abilities.
  • Collaborate closely with a multidisciplinary team to support patient care and recovery.
  • Guide patients through their recovery journey with compassion and professionalism.
  • Help restore patients' independence and well-being through high-quality therapeutic care.

Requirements

  • Graduate of an accredited Physical Therapy program

  • Current Pennsylvania Physical Therapy License

  • Minimum two years of physical therapy experience in Home Health or similar settings

  • Experience treating geriatric patients in a home health or similar care setting

  • Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting

  • Experience providing care in hospice-adjacent contexts

  • Valid Driver’s License and reliable vehicle

  • Proof of Auto Insurance

  • Strong communication skills for effective collaboration with patients, families, and the care team

  • Excellent organizational abilities and experience managing a diverse caseload

  • Eligible to work in the United States

  • Experience with Matrix Care is a plus.

Experience Physical Therapy: 2 years (Required) License/Certification: PA Physical Therapy License (Required) Benefits
