Full job description

NeuroRestorative, a part of the Sevita family, provides rehabilitation services for people of all ages with brain, spinal cord and medically complex injuries, illnesses and other challenges. In a variety of locations and community-based settings, we offer a range of programs, including vocational and therapy programs, day treatments, and specialized services for infants, children, adolescents, Military Service Members and Veterans.

Case Coordinator | NeuroRestorative

The Case Coordinator provides organizational assistance and program support while helping ensure participants receive the services, resources, and care they need to reach their goals and live as independently as possible in the community.

Monday–Friday | 8:30 AM–4:30 PM | 40 Hours/Week

Starting Pay: $16.25/hour

Experience Pay:

  • $0.50/hour for 1–4 years of experience

  • $1.00/hour for 5+ years of experience

  • Additional pay for education in a related field

Preliminary Requirements:

  • Valid non-probationary driver’s license held for at least 1 year

  • Ability to travel as needed to Galloway and Mays Landing OR Egg Harbor Twp. and Somers Point.

Make a Difference Every Day:

Provide direct participant care to adults with traumatic brain injury while supporting internal/external coordination of services.

What You’ll Do

  • Help ensure programs and services meet each participant’s individual needs

  • Provide support and assistance to designated program participants

  • Serve as a liaison with families, staff, and community resources

  • Organize and facilitate participant conferences

  • Review evaluations, progress reports, and discharge reports

  • Maintain accurate participant documentation and records

  • Coordinate and follow up on discharge planning

  • Support communication among team members regarding participant-specific needs

  • Attend clinical team and supervision meetings

  • Review monthly participant bills and identify discrepancies

  • Coordinate utilization with the Case Manager related to participant incentive accounts

Additional Responsibilities:

In this role, you’ll also help coordinate and support participants’ healthcare needs:

  • Organize and facilitate internal medical appointments

  • Track and ensure annual medical appointments are completed on time

  • Maintain and distribute appropriate medical documentation

  • Prepare medical appointment summaries and tracking information

  • Coordinate healthcare planning and follow-up

  • Review participant reports to ensure healthcare needs are addressed

  • Communicate with healthcare providers and community resources

  • Assist participants in understanding their healthcare needs

  • Help train new staff involved in overseeing medical care

Education & Experience

Required:

  • High School Diploma or equivalent

  • Two years of direct or indirect support services to individuals with disabilities in a medical and/or rehabilitation setting

Preferred:

  • Associate or Bachelor’s Degree in a Social Services-related field

Physical Requirements

This position involves light work, including occasionally lifting or moving up to 20 pounds.
