Position Summary

Mountains Community Hospital is seeking a compassionate, organized, and detail-oriented Case Management Assistant to support the Case Management Department in coordinating patient services, maintaining accurate documentation, and facilitating communication among patients, families, healthcare providers, and community resources.

Under the direction of the Case Management team, this position provides administrative and clerical support to assist with discharge planning, care coordination, referrals, resource identification, and other activities that promote continuity of care and effective patient transitions.

Essential Duties and Responsibilities

  • Provide administrative and clerical support to the Case Management Department.

  • Assist with discharge planning activities and coordination of patient services under the direction of the Case Management team.

  • Help coordinate referrals to post-acute care providers, home health agencies, skilled nursing facilities, rehabilitation services, and community resources as directed.

  • Assist with obtaining and organizing information needed for patient transitions and discharge arrangements.

  • Communicate with patients, families, healthcare providers, insurance representatives, and community agencies as appropriate.

  • Maintain accurate, timely, and organized documentation in electronic health records and departmental files.

  • Assist with tracking referrals, authorizations, discharge arrangements, and follow-up items.

  • Support the collection and organization of information related to patient placement, transportation, equipment needs, and community services.

  • Assist with obtaining information from outside facilities and agencies to support care coordination.

  • Protect confidential patient information and comply with HIPAA, hospital policies, and applicable privacy requirements.

  • Respond to telephone calls, route inquiries, and relay messages to appropriate Case Management personnel.

  • Assist with departmental reports, data entry, record maintenance, and other administrative tasks.

  • Perform other related duties as assigned.

Minimum Qualifications

  • High school diploma or equivalent required.

  • Previous administrative, clerical, healthcare, or customer service experience preferred.

  • Experience in a hospital, medical office, case management, discharge planning, or healthcare coordination setting preferred.

Knowledge, Skills, and Abilities

  • Strong organizational skills and attention to detail.

  • Effective written and verbal communication skills.

  • Ability to interact professionally and compassionately with patients, families, healthcare professionals, and community partners.

  • Ability to manage multiple tasks, prioritize assignments, and meet deadlines.

  • Basic computer skills, including word processing, data entry, email, and electronic record systems.

  • Ability to maintain confidentiality and handle sensitive patient information appropriately.

  • Ability to work independently while collaborating effectively with the Case Management team and other hospital departments.

  • Knowledge of medical terminology and healthcare processes preferred.

Work Schedule

This is a Per Diem position scheduled for 16 hours per week. The work schedule will be established based on departmental needs and operational requirements.
