Summary:

Responsible for assisting in management of overall operations of Regional Care Management

Department at UNC Health Rex and UNC Health Johnston. Provides direction and support to designated department leadership and teammates. Assists Regional Director in ensuring that the department achieves

its goals and objectives by engaging internal and external stakeholders in care coordination

and outcomes management processes. Accountable for departmental People Management,

Fiscal Management, Communication, Goals/Outcomes Management, and Quality/Process Improvement initiatives to ensure high level of quality consistent with organizational standards.

Reports to the Regional Director of Care Management.

Responsibilities:

  1. Communication - Develops and maintains positive relationships with key individuals internally and externally to enhance overall Care Management Departmental processes. Partners with external community agencies to ensure strong relationship for patient needs and support. Participate in community events and engage in community partnership opportunities.

  2. Customer Service - Assists Regional Director in establishing systems, processes, people, and education that will improve internal and external customer satisfaction and positive results for the department and institution. Serves as a role model in supporting team, departmental, and institutional expectations in customer service excellence.

  3. Fiscal Management – Collaborates with Regional Director to effectively establish and meet budgetary goals for Regional Care Management departments. Implements strategies to increase revenue and cost-effectively manage staff, supply, and equipment resources. Evaluates the impact of strategic fiscal changes on quality outcomes. Oversight of Patient Assistance Funds and Flex Funding activities. Monitors approved budget versus actual expenditures to reconcile as needed to achieve cost effective use of resources, includingtracking and trending of departmental productivity and variances across the region.

  4. Goals/Outcomes Management - Collaborates with Regional Director to establish goals for regional departments that are congruent with organizational goals. Reviews overall department performance to ensure teammates are meeting or exceeding target outcome goals. Determines and oversees successful implementation of optimum methods and process improvement to operationalize system-identified goals and objectives with key focus on monitoring length of stay index. Works closely with medical staff leadership and other staff to ensure appropriate involvement in care management efforts. Assists Regional Director in ensuring the collecting, analyzing, evaluating, measuring, and presenting of care management operational data. Uses data to take corrective action as needed to benefit the department and/or organizational outcomes.

  5. Accreditation Coordination – Ensures that internal and external regulatory standards and care management practices are met or exceeded for regional departments. This includes internal organizational policy and procedures as well as all external accrediting bodies. Ensures regional departments are prepared for accreditation visits, including auditing accreditation requirements and preparing department leaders for survey visits.

  6. People Leadership - Manages Complex Care Management Team for UNC Health System. Creates an environment conducive to recruiting and retaining staff. Applies organizational policy and procedures in hiring, promoting, transferring and terminating staff. Provides staff with annual performance feedback and opportunity to set professional goals. Provides oversight for all education and competencies requirements for teammates across regional departments. staffing levels (hiring/firing), workloads and productivity, staff development, regulatory compliance, continuing education, and performance appraisal.

  7. Quality & Compliance - Leads department specific quality improvement initiatives to ensure compliance with hospital regulatory requirements. Develops appropriate departmental policies and procedures oversees departmental auditing requirements. Ensures best practices for various departmental roles.

Education Requirements:

  • Master's degree in Nursing, Master's degree in Social Work (MSW), or Master's degree in related field.

Licensure/Certification Requirements:

  • Must have valid NC RN license or LCSW/LCSW A to practice in the state of NC within three years of hire.

  • ACM or CCM certification required within two years of hire.

Professional Experience Requirements:

  • Seven (7) years of experience in Care Management. Two (2) years of healthcare management experience. Will consider relevant combination of management and leadership experience.

Knowledge/Skills/and Abilities Requirements:

  • Knowledge of TJC, federal and state standards and health care operations Strong communication, software, project management, leadership, organizational, computer and analytical skills Ability to build and maintain collaborative relationships at all levels Critical thinking and problem resolution skills
