Delivering compassionate care to enhance lives and improve communities. We are seeking someone who enjoys coordinating with people, ability solve problems, organized and excellent communication skills. Does this describe you? If so, you may be the person we are looking for to join our dynamic Lake Havasu City team! Job Preview at a Glance: The Case Manager serves as a central support within the interdisciplinary team, working with children and adults. This staff member conducts assessments, assists members with resources within the community and provides coordination of care inside and outside of the agency. Responsibilities: - Intake screenings (Completing intake paperwork and referrals) - Completing annual behavioral health assessments, including service planning and support planning - Facilitating CFTs and ARTs - Facilitating groups - Identifying gaps in care for our members - Completing SMI and SED evaluation
