Full job description

JOB REQUIREMENTS: Provide individual, group and family therapy, as well as occasional crisis counseling, in both a clinic and community setting. Community Services Division ***** OTHER EXPERIENCE AND QUALIFICATIONS: Active WI Medical Assistance Provider Number within 90 days of hire; insurance acceptable driver's license; Qualify under Wisconsin Caregivers Law, Wisconsin Act 27.
