Supports, participates and works cooperatively with the director of staffing. 2. Responsible for overall quality of patient care and clinical performance of nurses and aides. 3. Organizes clinical operations for the care center. 4. Reviews requests for services and determines patient eligibility/suitability for home care services. Works with intake in coordinating patient referrals and admissions. 5. Oversees scheduling. 6. Oversees payor verification and precertification requirements. 7. Reviews documentation of other staff members and ensures missing, incomplete and/or untimely documentation issues are resolved. 8. Assists clinicians in establishing immediate and long-term patient goals, setting priorities and developing Plan of Care (POC). 9. Regularly evaluates the needs of the patient and makes necessary revisions to the plan of care in collaboration with clinicians. 10. May provide direct patient care on an infrequent basis in times of emergency. 11. Participates in the on-call process. 12. Assists with training of staff to ensure quality care, compliance and fiscal responsibility. 13. Participates in the investigation and resolution of patient/family/physician concerns. Qualifications 1. Associate or bachelors degree in nursing. 2. One year of clinical RN experience in home health or hospice. 3. One year of RN management experience. 4. Current RN license, specific to the state(s) you are assigned to work. 5. Current CPR certification.
