About the Role: ComForCare Home Health Care - Hilton Head is looking for a compassionate and skilled Home Health LPN to join our dedicated care team in beautiful Hilton Head, SC. Responsibilities:

  • Monitor and document patient health status, vital signs, and progress toward care goals

  • Communicate effectively with patients, families, and the interdisciplinary care team

  • Educate patients and caregivers on disease management, medications, and healthy practices

  • Maintain accurate and timely clinical documentation in compliance with home health regulations

  • Identify changes in patient condition and report to supervising RN or physician promptly Requirements:

  • Active LPN license in the state of South Carolina

  • Prior home health, long-term care, or clinical nursing experience preferred

  • Strong knowledge of medication administration, wound care, and patient assessment

  • Excellent communication and interpersonal skills with patients and families

  • Reliable transportation and a valid driver's license for home visit travel

  • Ability to work independently and manage time effectively in a field-based role
